Medical forms: STUDENTS

A student admitted in the first year of study (undergraduate, graduate, exchange, visiting, NGA, etc.), who has recovered after expulsion or returned from academic leave (1 year or more), as well as applying for residence in the AUCA Dormitory, is required to fill out and submit to the Medical Service Office the following documents:

1. Medical form “Student Health Information” >>
2. Certificates, reports of medical specialists, test results specified in the “List of Medical Documents” >>

All documents are required to be submitted to the Medical Service Office within the following deadlines:

• for a student admitted for the first year of study - within 10 working days from the date of signing the “Agreement on Educational Services”;
• for a student returning from academic leave - on the day of application for readmission;
• or a student applying for residence in the AUCA Dormitory - on the date of application for a place to reside in the AUCA Dormitory.

Medical documents are submitted to the Medical Service Office in Kyrgyz, Russian, or English, only one copy, in one language version!  In case the documents are registered in other languages, a notarized translation must be attached. Original medical reports must be certified by a legible stamp of the medical institution, stamp, and signature of the doctor.

Each year, in September, or within other deadlines established by AUCA order, all AUCA students must get fluorography arranged by AUCA. 

In case an AUCA student misses fluorography arranged by AUCA, he/she shall get fluorography on his/her own within 1 week and submit to the Medical Service Office a report/certificate by the relevant medical specialist with the results of the examination. AUCA has the right to apply administrative measures to those who violate this rule.
